Initially you must submit either A or B: A) A negative two-step PPD with the read dates between 7-42 days apart. Your PPD must be read 2-3 days after it is implanted. B) A negative QuantiFERON/T-Spot Annually thereafter, you must submit either C or D: C) A negative Annual PPD D) A negative QuantiFERON/T-Spot If you test positive for tuberculum exposure, you must submit E: E) A negative Chest X-Ray and TB Questionnaire filled out by a healthcare provider. Your chest x-ray will explire every 5 years and your TB Questionnaire will expire every year. |
CDC Guidance on Hepatitis B Vaccination
You must submit a positive Hepatitis B Titer
A) Three doses of Hepatitis B and a positive Hepatitis B Surface Titer B) Two doses of Heplisav (2-Shot Series) and a positive Hepatitis B Surface Titer If your titer is non-immune, you may submit Repeat Hepatitis B Doses and a Repeat Hepatitis B Surface Titer. You may also submit the Hepatitis B Waiver Form, instead of the repeat doses and titer. If you are completing a Hepatitis B vaccine series for a second time, you will be temporarily compliant per the below timeline as you upload each dose: Hepatitis B Dose 1 = 30 days |

CDC Guidance on Influenza Vaccination
You must submit proof of having received an influenza shot every year. If you choose not to receive the flu shot, you may be required to wear a mask onsite during your clinicals. You must also electronically sign and submit the Flu Shot Declination. This is an annual requirement and is only tracked during a certain timeframe. |
